Filing a police report is important if you have been a victim of ID theft since it will give your creditors documented proof of the crime.

This letter can be used as a follow-up to telephone notification to law enforcement officials that you have been a victim of identity theft. You may wish to send copies of this to both your local Police Department and the Police Department in the city where the identity theft occurred. Since identity theft is a federal crime, you should also send a copy to the Fraud Department of the Secret Service at:
